About the Mid Semester Site Visit

The purpose of this assignment is to assess the student’s placement during the course of the semester so that actions may be taken to address any issues as soon as possible.
The completion of this form will prepare the student and their Faculty Liaison for a scheduled mid-semester meeting with you.
Students are asked to critically assess their placement, practice opportunities, and experiences to date and complete this form to share with the Faculty Liaison only.
If there are any issues that need to be addressed, the Faculty Liaison will make a plan with the student to inform you of these issues and work toward a resolution.
