GSA P-Card Checkout Form

This form is used only when physically checking out the GSA P-Card from the GSA Office for an approved purchase.

⚠️ This is NOT P-Card training and does not provide instructions on how to use the card. Required training and approvals must be completed before coming to the GSA Office. Please email gsaclubs@unr.edu to get the link for training.
